Following an inquest into the death of Santosh Benjamin-Muthiah, who died in a fire in 2010, Coroner Andrew Walker has accepted recommendations put forward by Hertfordshire Trading Standards and London Fire Brigade.
Sarah Simcock, Edward Pleeth and Jemma Lee were all instructed on behalf of the London Fire and Emergency Planning Authority.
